Please bring in all signed documents when you visit us for the first time. Doing so will reduce your wait time as well as errors in our system. Please plan on arriving 15 minutes before your scheduled appointment time with your I.D. and insurance card. All patients under the age of 18 MUST have a parent or legal guardian with them.
