We utilize our Patient Portal for you to update your information and medical history, and upload a copy of your photo ID and insurance cards prior to your visit. Completing this before your appointment will ensure a smooth and easy Contactless Check-In process in our office.
If you need to register, please call our office and have a new myPatientVisit link text/emailed to you. (Link expires after 72 hours.) All information provided is confidential, secure, and HIPAA compliant. If you are already registered with MyPatientVisit.com and need to connect to our practice, our Practice ID is POQSKQ.

We accept most major Insurances. However, it is the responsibility of the patient to verify whether we are In-Network with your Insurance Plan. You may call your Insurance Provider and give them our Tax ID #58-1846036 to verify your In-Network Benefits.
As a courtesy to our patients, we file to most insurance companies. This does not imply in-network coverage or benefits. Please utilize the Patient Portal for all Prescription questions and refill requests. Simply send us a message with your requests and our Triage staff will work on your request appropriately.
Please note, if it has been more than 1 year since your last visit with your provider, we will not be able to refill your prescription. All patients must be seen at least once a year in order to refill any prescriptions. Our appointment concierge can assist you with getting scheduled.

Appointment Reminders & No-Show:
Dermatology Consultants gives a courtesy reminder call or text of your choice for appointments. It is the responsibility of the patient to show for all appointments scheduled. Missed appointments will incur a $50 fee. Twenty-four hour notice is required to cancel or reschedule an appointment without a fee. Some Aesthetic, Laser & Body Sculpting Services require a Non-Refundable deposit.
Dermatology Consultants, P.C. utilizes an outside lab facility, PathGroup, for blood work, biopsies, cultures, etc. An employee from PathGroup resides in each location and operates as the phlebotomist and lab technician. These technicians are not employees of Dermatology Consultants, P.C. The employees are employed by PathGroup and all directives are given from PathGroup. Insurance/billing is handled separately by PathGroup. You will receive a separate explanation of benefits from your insurance carrier. You may also receive a separate bill from the lab, depending on the benefits of your plan.
